ACDS MDS (PG-WAT) Application Form 2021

PG-WAT 2021 is going to be conducted by ACDS (Army College of Dental Sciences) in the month of March 2021 for admission to MDS programmes at ACDS, Secunderabad. Interested and eligible candidates are required to apply for the same on or before February 2021. We are sharing below all important information related to the PG-WAT 2021 application procedure:

• Do not put any fake information in the application form. If you have knowingly putted wrong info, your admission is liable to be rejected by the respective authority.
• After filling up your application, affix passport size photograph on it. An application without affixing the photograph is liable to be rejected by the respective authority.
• The institute is not going to accept the application form received after 15^th February, 2021 without late fee.
• Candidates are instructed to send their applications through speed post or registered post with A/D. Private courier service may delay in submission of the form.

*Candidates who have made changes in the communication address after filing the application form they must need to check with ACDS on regular interval of time for latest updates regarding the same.

List of Documents Need to be Enclosed in the Application Form

• Xerox copy of Registration certificate obtained from the respective State Dental Council.
• Xerox copies of all mark sheets starting from first BDS to final BDS.
• Copy of Experience/Internship certificate.
• Copy of provisional or original BDS certificate.
• Migration certificate
• Transfer certificate
• Character certificate of the college from where candidates have obtained their graduation degree.
• Copy of study certificate of BDS program.
• Candidates, who are already studying MDS course they need to submit the Discontinuance certificate, should be issued by the respective Principal.
• Correspondence certificate.
• GAP Affidavit
• Medical certificate.
• Candidates’ and their parents’ undertaking needs to be produced at the time of counseling.
• Undertaking certificate for women candidates showing she is unmarried and is a dependent ward of retired/serving army personnel.
• Undertaking certificate for Male candidates showing he is not employed in any organization/institution and completely dependent ward of retired/serving Army personnel.
• Dependency Certificate.
• 8 Stamp size and passport size photographs need to be submitted at the time of counseling.
